If you are living away from home on the College Campus, the College of St Mark & St John strongly advise you to register with a local doctor unless you are already registered with one within easy reach of your accommodation. The College recommends Elm Surgery.

Once you have received confirmation of your place, and have received your accommodation address on the campus you register with Elm Surgery. Click here for a registration form which you should complete, sign and post to us at Elm Surgery, 123 Leypark Walk, Estover, Plymouth PL6 8UF. At the same time please click here for a New Patient Health Questionnaire which helps us to get to know you a little better. Please complete and return the questionnaire with your registration form.

When you move from the College Campus you may need to register with a surgery closer to your new address. Student Services at the College will provide you with further information when the time comes.
